Responsibilities
As an LPN, you will:
- Perform comprehensive nursing assessments for AFC and GAFC participants.
- Complete and maintain MDS-HC (Minimum Data Set–Home Care) assessments accurately and on schedule.
- Monitor participants' health status and identify changes in condition.
- Administer medications and provide skilled nursing care as needed, including wound care and chronic disease monitoring.
- Document nursing visits, assessments, and progress notes in a timely and accurate manner.
- Communicate significant clinical findings to supervising Registered Nurses, physicians, and interdisciplinary team members.
- Assist in developing, implementing, and updating individualized care plans.
- Provide clinical guidance and support to caregivers and direct care staff.
- Educate participants and caregivers on disease management, medication safety, and healthy living.
- Ensure compliance with Mass-Health, Massachusetts Department of Public Health, AFC, and GAFC regulations.
- Participate in quality improvement initiatives and emergency preparedness activities.
Qualifications
- Current Massachusetts Licensed Practical Nurse (LPN) license in good standing.
- Minimum of one year of nursing experience preferred.
- Experience in home care, community health, long-term care, AFC, GAFC, or similar settings preferred.
- Experience completing MDS-HC assessments is highly desirable.
- Strong clinical assessment and documentation skills.
- Excellent communication, organizational, and interpersonal skills.
- Ability to work independently while collaborating with an interdisciplinary team.
- Reliable transportation and willingness to travel within assigned service areas.
- Current CPR certification preferred.
