Loved Ones Care is seeking compassionate and dependable Direct Care Aides and Home Health Aides to support adults in their homes.
The DCA/HHA assists members with daily activities that help them remain safe, comfortable, and independent in familiar surroundings. This may include personal care, homemaking, meal preparation, mobility assistance, toileting, transportation, shopping, and companionship.
The ideal candidate is caring, reliable, respectful, and able to work independently while providing high-quality, person-centered care.
Responsibilities
- Assist members with activities of daily living, including bathing, dressing, grooming, toileting, mobility, and continence care.
- Provide homemaking services such as light housekeeping, laundry, meal preparation, and shopping.
- Support members with safe transfers, ambulation, and mobility as directed by the care plan.
- Provide reminders and prompts for meals, hydration, toileting, and other daily needs.
- Offer companionship, social interaction, and emotional support.
- Observe changes in the member's physical, mental, or emotional condition.
- Report concerns promptly to the supervisor or clinical team.
- Complete visit notes and Electronic Visit Verification documentation accurately and on time.
- Follow the member's individualized care plan and agency policies.
- Maintain a safe, clean, and respectful home environment.
- Protect member privacy, dignity, rights, and confidentiality.
- Follow infection-control, emergency-preparedness, and safety procedures.
- Provide transportation or accompany members to appointments and errands when authorized.
- Work collaboratively with members, families, caregivers, nurses, and care coordinators.
Requirements
- High school diploma or GED preferred.
- Massachusetts CNA, HHA, Personal Care, Homemaker, or other accepted direct-care certification, as applicable to the position.
- Previous home care, personal care, or direct-care experience preferred.
- Compassionate and respectful approach to working with older adults and individuals with disabilities.
- Excellent communication, organizational, and time-management skills.
- Ability to work independently with limited supervision.
- Reliable transportation, valid driver's license, safe driving record, and current automobile insurance when travel is required.
- Ability to complete required background checks and pre-employment screening.
- Ability to use a smartphone or mobile application for visit notes and Electronic Visit Verification (EVV).
- Ability to safely perform the physical duties of the position.
