As an RN, you will play a vital role in helping individuals remain safe, healthy, and independent in their homes while ensuring compliance with Mass-Health and Adult Foster Care regulations.

Position Summary

The Registered Nurse serves as the clinical leader for assigned AFC and GAFC members. Responsibilities include conducting comprehensive nursing assessments, completing MDS-HC assessments, developing and updating care plans, supervising LPNs and caregivers, ensuring regulatory compliance, and collaborating with healthcare providers to coordinate quality care.


Responsibilities

As an RN, you will:

  • Complete comprehensive nursing assessments for AFC and GAFC members.
  • Perform MDS-HC (Minimum Data Set–Home Care) assessments and reassessments in accordance with MassHealth requirements.
  • Develop, implement, and update individualized care plans.
  • Conduct initial, monthly, quarterly, semi-annual, annual, and change-in-condition nursing assessments.
  • Supervise Licensed Practical Nurses (LPNs), caregivers, and direct care staff.
  • Review nursing documentation to ensure accuracy, completeness, and regulatory compliance.
  • Monitor members with chronic illnesses and identify changes requiring intervention.
  • Coordinate care with physicians, hospitals, rehabilitation facilities, home health agencies, behavioral health providers, and community organizations.
  • Educate members and caregivers regarding medications, chronic disease management, fall prevention, nutrition, infection prevention, and wellness.
  • Participate in interdisciplinary team meetings and case conferences.
  • Review medication profiles and assist with medication reconciliation.
  • Ensure compliance with MassHealth, Executive Office of Health and Human Services (EOHHS), Department of Public Health (DPH), AFC, and GAFC regulations.
  • Assist with quality assurance, utilization review, and clinical audits.
  • Participate in emergency preparedness planning and member safety initiatives.
  • Support program growth through clinical consultation during admissions and referrals.

Qualifications

Required

  • Current Massachusetts Registered Nurse (RN) license in good standing.
  • Minimum of one year of clinical nursing experience.
  • Strong assessment, critical thinking, and clinical documentation skills.
  • Excellent communication and organizational skills.
  • Ability to work independently while managing multiple priorities.
  • Reliable transportation and willingness to travel throughout assigned service areas.
  • Current CPR certification.

Preferred

  • Experience in Adult Family Care (AFC), Group Adult Foster Care (GAFC), home care, community health, case management, hospice, long-term care, or behavioral health.
  • Experience completing MDS-HC assessments.
  • Knowledge of MassHealth regulations and AFC/GAFC standards.
  • Experience supervising nursing staff and caregivers.
  • Familiarity with electronic health record (EHR) systems and EVV documentation.
